Health Assessment

What Are Health Assessments?

Health Assessments (PDF, 39 KB) communicate the health concerns and information from parents and physicians for all children entering public schools for the first time in North Carolina. Health issues that affect learning are addressed.

State law requires that every child entering public schools in N.C. receive a health assessment. The assessment must occur within 12 months prior to entering school. The medical provider, parent or guardian must provide a completed health assessment transmittal form to the principal of the school on or before the child's first day of attendance.

Many local health departments and physicians across the state perform the assessment.

Who is Eligible

All children are required to receive a Health Assessment prior to entering school.

How to Apply

Your child can get a health assessment at a participating local health department or at your doctor’s office. Call for an appointment. Payment may be determined by income and insurance type.
