MA-2165 outlines instructions for the Qualifying Individuals # 2 (QI2) group. QI2s are entitled to reimbursement of a portion of their Medicare Part B premium. For 2002, that portion is $3.91 per month for an annual maximum payment of $46.92.
Reimbursement is 100% federally funded. DMA, in conjunction with the DHHS Controller’s Office, will again issue QI2 checks. One check will be written and mailed by the state. It will include benefits for all months in 2002 that the individual was eligible for QI2. (Attached is a chart showing the payment amounts based on the number of months of QI2 eligibility.)
The purpose of this letter is to provide the deadlines for submitting information to DMA for the issuance of a check to each individual determined eligible for QI2 benefits for 2002.
Congress has not taken action to continue funding for the QI2 program beyond December and the State cannot continue to fund the program due to budget constraints. As a result, the Qualifying Individuals QI2 program will end on December 31, 2002.
You may use the paper copy of the QI2 Check Register (MA-2165, Figure 3) or an Excel spreadsheet of the Check Register. If you have the capability, DMA recommends using the Excel Check Register. Note: If you received the Excel Check Register last year, you may use it again. It has not changed.
Submit the check register to DMA no later than November 22, 2002. Retain a paper or electronic copy of the check register for your records. Submit a QI2 register even if there are no individuals approved for QI2. It is very important to submit the check register timely in order to issue QI2 checks before the Christmas holidays.

NOTE: For confidentiality purposes, if you fax the register you must include the following statement on the fax cover sheet:
“This fax is confidential and intended solely for the use of the individual or entity to whom it is addressed. If you have received this fax in error please notify the sender and destroy this cover sheet and its attachments.”
After you submit your check register to DMA, begin a Straggler QI2 Check Register (DMA-2165, Figure 4) listing any applications approved for QI2 for 2002 after the first check register was submitted.
All pending QI2 applications for 2002 must be completed by January 16, 2003. Submit the Straggler QI2 Check Register by January 17, 2003, by e-mail to Susan Ryan or mail or fax to Medicaid Eligibility Unit as instructed in II. above.
1. The DHHS Controller’s Office will mail a check to each QI2 recipient based on the information on the QI2 Check Register.
a. Checks for individuals submitted on the first check register will be mailed about December 15, 2002.
b. Checks for individuals submitted on the straggler list will be mailed in mid February 2003.
2. Included with the checks will be a Notice of Your QI2 Medicare Savings Program is Ending. A copy is attached to this letter.
