I. GENERAL INFORMATION

    Use Application Inquiry to view any application that has been registered. Applications are registered on-line using the DSS-8124 and are available for immediate inquiry. Information remains on file once the application is dispositioned.

    There are three different ways to perform an application inquiry. You can inquire by application number, Individual ID, and social security number. Use the following instructions to perform these inquiries.

    Application data may also be accessed using the Name Search function. Refer to EIS 1056 for instructions on how to perform this inquiry.

II. ACCESS PROCEDURES

    A. Application Number Inquiry

      1. From the EIS Inquiry Menu, key “AD” in the SELECTION field.
      2. Key the seven digit application number followed by a “G” in the KEY field. Press ENTER.
        a. When the application is pending, the following screen appears displaying the most current application data for the case.

        b. If a two part application has one part already dispositioned, press PA-1 as indicated at the bottom of the screen to view the second page of the inquiry for the pending application.

          NOTE: IF THE APPLICATION HAS BEEN PLACED ON HOLD EITHER BY ERRORS OR BY WORKER, “HD” APPEARS IN “STATUS”.

            IF THE APPLICATION IS ON HOLD DUE TO A SSI ACTION, “HX” APPEARS IN “STATUS”.

        c. When the application has been disposed, the following screen appears displaying the most current application data for the case.

          NOTE: EIS displays the most current EPICS Claim Indicator in the CLM field for each individual. THE CLAIM INDICATOR MAY CHANGE EACH TIME AN INQUIRY IS PERFORMED.

    B. Individual ID - Application Inquiry

      1. Key “AD” in the SELECTION field.
      2. Key the 10 digit Individual ID in the KEY field.
      3. Press ENTER.

    The following screen appears displaying the most current application data for that Individual ID.

      4. Key an “S” in the SEL field beside the form ID number to display more detailed application data. Press ENTER.
        a. When the application is pending, the following screen appears displaying the most current application data.

          NOTE: IF THE APPLICATION HAS BEEN PLACED ON HOLD WITH ERRORS OR BY THE WORKER, “HD” APPEARS IN “STATUS”.

              IF THE APPLICATION IS ON HOLD DUE TO A SSI ACTION, “HX” APPEARS IN “STATUS”.

    C. Social Security Number - Application Inquiry

      1. Key “AD” in the SELECTION field.
      2. Key the nine digit social security number (SSN) in the KEY field.
      3. Press ENTER.

        The following screen appears displaying the most current application data for that SSN.

      4. Key an “S” in the SEL field beside the form ID number to display more detailed application data. Press ENTER.
        a. When the application is pending, the following screen appears displaying the most current application data.

          NOTE: IF THE APPLICATION HAS BEEN PLACED ON HOLD WITH ERRORS OR BY THE WORKER, “HD” APPEARS IN “STATUS”.

            IF THE APPLICATION IS ON HOLD DUE TO A SSI ACTION, “HX” APPEARS IN “STATUS”.

        b. When the application has been disposed of, the following screen appears displaying the most current application data for the case.
