EIS 3552 – POSSIBLE DUPLICATE ID REPORTS

A priority objective of the Eligibility Information System is to identify uniquely each case and individual in the system. Completing a Social Security Number Search and Name Search on each applicant helps to ensure that a previously assigned Case or Individual ID number is reassigned at reapplication. If the Social Security Number Search and the Name Search are not performed correctly, duplicate Case and/or Individual ID's may be assigned to the same case/individual.

Use the DSS-8128, POSSIBLE DUPLICATE ID RESOLUTION REPORT, to combine duplicate Individual ID's. Instructions for completing both the county initiated and the State generated DSS-8128 are located in EIS 3300, Resolving Duplicate ID's.

When a DSS-8128 is completed and keyed into the Eligibility Information System, the ERRORS AND UPDATES FROM 8128 UPDATE REPORT is produced. It is printed at the State Office and mailed to the county the next workday. Check each report for messages when received. Ensure the DSS-8128's are corrected, if necessary. Refer to EIS 3300, Resolving Duplicate ID's.
