The Breast and Cervical Cancer Medicaid (BCCM) case management report, DHREJ BREAST/CERV CERT ENDING REPORT, determines when BCCM cases are due for review. This report will become a part of the regular County Case Management Report, DHREJ CASE MANAGEMENT REPORT. This change is a result of county requests.
Beginning with the report run on October 29, 2004, for November 2004, the Case Management Report will now contain the BCCM cases due for review. The Eligibility Information System (EIS) will not produce review appointment notices for these cases but will produce address labels.
A. MA-3250, Breast and Cervical Cancer Medicaid, is changed to reflect the inclusion of the BCCM cases on the Case Management Report.
B. The withdrawal/denial notices procedure is expanded to include sending a copy of the notice to the Breast and Cervical Cancer Control Program (BCCCP) Coordinator.
1. DMA-5081, Verification of Screening, Diagnosis, and Treatment, has been modified to clarify the applicant’s enrollment per the NC Breast and Cervical Cancer Control Program (BCCCP) guidelines.
2. DMA-5081R, Recertification for Continuing BCCM Eligibility, has been developed to use during reenrollment.

3. DMA-5079, Breast and Cervical Cancer Medicaid Application, has been modified to clarify the applicant’s enrollment per the NC BCCCP guidelines and to include changes to options under the race/ethnicity questions.
This policy change is effective November 1, 2004.
