SUMMARY OF SERVICES AND REPORTING CODES
Services used for individual consumer service planning and codes used for reporting are summarized below. Please refer to respective Manual sections for a complete definition of each service provided through the Independent Living Services Program.
ADJUSTMENT SERVICES FOR THE BLIND AND VISUALLY IMPAIRED
Code 360 - includes the functions of service planning, service delivery and case management of services for persons who are blind and visually impaired. Code 360 is also used to reflect the planning aspects of In-Home Aide Services: Level I-Home Management.
Code 360 is also used by the SWB to report skills development training in meeting the demands of daily living. Also, the provision of adaptive aids and equipment.
Code 360 is also used by the SWB to report skills development training through camping experiences at Camp Dogwood, local recreation programs, and arranging for adjustment training at the Rehabilitation Center in Raleigh.
HEALTH SUPPORT SERVICES
Code 110 - counseling regarding health problems
INDIVIDUAL & FAMILY ADJUSTMENT SERVICES
Code 330 - counseling of individuals and their families on problem areas of the consumer
IN-HOME AIDE SERVICES: LEVEL I-HOME MANAGEMENT
Code 350 - the purchase of In-Home Aide Services: Level I: Home Management only. Must be planned and recorded in conjunction with Code 360 which reflects the planning aspects of Level I: Home Management
