Hearing Impairment Codes Used on Register

B/VI Due to Retinitis Pigmentosa With:

B/VI Due to Other Causes With:

Additional Instructions:

If person does not normally use a hearing aid, base your hearing evaluation on unaided loss. If person commonly uses a hearing aid, evaluate with aid.

Mild - has hearing loss but is able to understand speech in all environmental settings (exception would be a large noisy room)

Moderate to Severe - may hear speech in quiet one-on- one setting, but has difficulty with other environments.

Profound - may hear sounds but does not understand speech.

(Codes 2-8 will be deaf-blind/visually impaired. The codes 1 and 9 will be hearing-impaired, blind/visually-impaired and should be followed through the years)
